Where is a forty foot fall possible except the climb out? There's only one rap and it's more than 40 feet. The downclimbs are all 10-15 feet as I recall.
Anywhere along the rim near the first rappel. There are 2 entry options, the direct, and the big rap on the left rim. Perhaps he fell near the beginning.
Danny Neds posted on Fecebook that he heard from friends who did Yankee Doodle yesterday that SAR was in the vicinity of Boltergeist. Maybe the media is... Mistaken? It's too early to know much. Less than twenty four hours? Let's give it some time. Condolences to the individual's family and friends.

My heart felt sympathies go out to all that are impacted by this tragedy. To the four people who's lives will be forever changed, to SAR for their tireless efforts, to the friends and family of the person that we lost, and to community as a whole that grieves each time we read about these events....
